Terms and Conditions for Switching to Electronic Account Statements (eStatements):

By clicking “Submit” on this form, I acknowledge and agree to the following terms and conditions:

• I wish to receive account statements in electronic format (eStatements) via online access through the UOB TMRW mobile application, and I request to cancel the delivery of paper statements by mail for all eligible financial products and services I hold with United Overseas Bank (Thai) Public Company Limited, including UOB Credit Cards, UOB CashPlus accounts, deposit accounts, mutual fund accounts, and others.
• If I do not submit a request to change my statement delivery method through this form, the Bank will continue to send my account statements in the format I previously selected.
• Monthly mutual fund statements issued by the Bank will not be combined with the unit balance reports issued by the asset management company (AMC), such as statements for tax‑deductible mutual fund accounts.
• If I provide incomplete or inaccurate information that does not match the information previously registered with the Bank, I acknowledge and agree that the Bank will be unable to update my statement delivery method to electronic format. In such cases, I will continue to receive account statements in the format previously selected for each financial product and service I hold with the Bank.
